Payment Voucher for Individuals
Form 1040-V is a payment voucher you send with a check or money order when you owe tax on your 2025 Form 1040, 1040-SR, or 1040-NR. It tells the IRS whose payment it is and how much you are paying. You only use it when you pay by mail; the form says not to complete it if you pay online.
The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

How to fill out Form 1040-V
Check that you need the voucher
Use Form 1040-V only when you send a payment by check or money order. The form says that if you pay online at irs.gov/Payments, you do not complete it.
Lines 1 and 2: Social security numbers
On line 1, enter your SSN. On a joint return, enter the SSN shown first on the return. On line 2, enter the SSN shown second on a joint return. If you have an ITIN, enter it wherever an SSN is asked for.
Line 3: Amount you are paying
Enter the amount you are paying by check or money order.
Line 4: Names and address
Enter your first name, middle initial and last name. On a joint return, also enter your spouse's name. Enter your home address, apartment number, city, state and ZIP code exactly as shown on your return. If you have a foreign address, also fill in the foreign country name, province or state, and postal code.
Prepare your check or money order
Make it payable to "United States Treasury". Make sure your name and address are on it, and write your daytime phone number, your SSN (the first SSN on a joint return) and "2025 Form 1040", "2025 Form 1040-SR" or "2025 Form 1040-NR". Write the amount on the right side as $ XXX.XX, without dashes or lines.
Cut out the voucher
Cut along the line marked "Separate here" on page 1. Put the voucher, your payment and your return loose in the envelope. Do not staple or attach them to each other.
After you fill it out
The form says to mail your 2025 tax return, payment and Form 1040-V together to the IRS. Page 2 of the form lists the mailing addresses for payments by state and for people abroad; check the current address on the IRS where-to-file page before you mail. The IRS asks that you use regular mail for a payment sent with Form 1040-V, because a private delivery service may delay processing.
Signing: Form 1040-V has no signature line, so the voucher itself is not signed.
Watch out for
- Do not staple or otherwise attach the voucher or your payment to your return or to each other. Put them loose in the envelope.
- If you pay online at irs.gov/Payments, do not complete or mail this voucher.
- When you pay by check, the IRS may use the check's information to make a one-time electronic funds transfer. If it does, money can leave your account as soon as the day the IRS receives the payment, and your bank will not return the check to you.
- Do not send cash with the voucher. The IRS has a separate in-person cash payment option, described on the form and on irs.gov/Payments.
