Application for a U.S. Passport for Eligible Individuals: Correction, Name Change to Passport Issued 1 Year Ago or Less, and Limited Passport Replacement

Form DS-5504 is the State Department's mail-in application for a new U.S. passport book or card when a recent passport needs to be corrected or replaced. It covers three cases: your name changed by marriage or court order less than a year after your passport was issued and the passport is less than a year old; your identifying information was printed incorrectly; or your passport book was issued at the full fee and limited to two years or less for a reason other than multiple losses or serious damage. The form says there is no fee unless you ask for expedited service or 1-2 day delivery for a name change or limited passport replacement.

The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

Who uses Form DS-5504

  • People whose name changed by marriage or court order less than one year after their passport was issued, when that passport is less than one year old
  • Passport holders whose identifying information, such as name, sex marker or place of birth, was printed incorrectly, or whose passport has a printing error such as missing data or crooked printing
  • Holders of a passport book issued at the full passport fee and limited to two years or less, for a reason other than multiple losses or serious damage
  • Parents or legal guardians, for a child whose passport meets one of these conditions
  • Holders of no-fee regular, service, official or diplomatic passports, who must first ask their sponsoring agency how to route the application

Before you start

  • Your most recent U.S. passport book and/or card. You must send it with the form.
  • One color photo, 2x2 inches, taken in the last six months, on a plain white or off-white background, without eyeglasses
  • For a name change: a certified name change document, such as a certified marriage certificate, divorce decree or court order, showing a seal and the official's or judge's signature
  • For a correction: a document that shows the correct information, such as a certified birth certificate
  • For a limited passport: evidence of U.S. citizenship (such as a birth certificate or naturalization certificate) and/or identity (such as a driver's license or state ID)
  • Your Social Security number. If you were never issued one, enter zeros and attach the signed and dated statement described in the item 5 instructions.

How to fill out Form DS-5504

  1. Check that you can use this form (Section A)

Instruction Page 1 lists three yes/no statements: a recent name change on a passport issued less than a year ago, information printed incorrectly, or a book limited to two years or less. If you answer no to all three, you cannot use this form. If your passport was lost, stolen, damaged or mutilated, the instructions say to apply on Form DS-11.

  2. Choose the document and fill in items 1 to 9

At the top of Application Page 1, pick a passport book, a passport card or both, and a regular or large book. Item 1 is the name to appear in the passport, item 2 your date of birth (MM/DD/YYYY), item 3 your sex marker (M or F, following the item 3 instructions), item 4 your place of birth, item 5 your Social Security number, item 6 your email, item 7 your primary phone number, item 8 your mailing address, and item 9 up to two other names you have used (attach a sheet for more). A passport card holds at most 24 characters for your first and middle names; if yours are longer, shorten one in item 1.

  3. Item 10: your most recent passport, and the photo

Enter your name exactly as printed in your most recent passport, then the book number and issue date and/or the card number and issue date. Staple your photo in the photo box with four staples placed vertically in the corners, as close to the edges as possible, without bending the photo.

  4. Sign and date Application Page 1

Read the declaration, then sign and date in the designated area. An applicant age 16 or older signs the applicant line. The parent or legal guardian line is marked for use when identifying a minor. Leave the boxes marked for the issuing office blank.

  5. Items 11 to 19 on Application Page 2

Repeat your name and date of birth at the top. Then give your height (11), hair color (12), eye color (13), occupation if you are 16 or older (14), employer or school if any (15), other phone numbers (16), a permanent address only if your mailing address is a P.O. box or you live somewhere else, with no P.O. box (17), an emergency contact who is not traveling with you (18), and your travel plans, or "none" (19).

  6. Items 20 to 23: the reason you are applying

Items 20 and 21 cover a name change: answer yes or no, and if yes, write your new name. Item 22 covers a misprint: answer yes or no, check only the boxes for the fields that need correcting, and write them as they should appear. Item 23 asks whether your most recent book was limited to two years or less; the book you send must not be expired.

After you fill it out

Print Application Page 1 and Application Page 2 on separate sheets of paper. Mail them with your current passport, your photo and your supporting documents to the National Passport Processing Center at the address on Instruction Page 3; routine and expedited service use different addresses, and expedited mail should say "Expedite" on the outer envelope. Check current fees on the State Department's website, and use a trackable mailing service, as the Department recommends. If you live outside the United States and Canada, you cannot use the domestic addresses; follow your U.S. embassy or consulate's procedures.

Signing: The form says to use black ink only and that you must sign and date the application in the designated area. Print the form, then sign and date Application Page 1 by hand in black ink.

Watch out for

  • If you make an error, the form says to complete a new one. Do not correct or white out entries.
  • If your passport was lost, stolen, damaged or mutilated, you cannot use this form. The instructions say to apply on Form DS-11.
  • Print the two application pages on separate sheets of paper, not on one double-sided sheet.
  • U.S. citizens living outside the United States and Canada cannot mail this form to the domestic addresses. Each U.S. embassy and consulate has its own procedure.

Questions about Form DS-5504

Is there a fee for Form DS-5504?

The form says there is no fee unless you request expedited service or 1-2 day delivery for a name change or limited passport replacement. A passport with a data or printing error is corrected at no cost, with no expedite fee, if it is still valid. Check current fees on the State Department's website.

How long is a corrected passport valid?

If you report the error within one year of the issue date, the new passport is valid for 10 years for an adult or 5 years for a minor. If you report it later, the new passport is valid only until the original one would have expired.

My name changed more than a year after my passport was issued. Can I use this form?

Not for the name change. This form covers a name change only when it happened less than a year after the passport was issued and the passport is less than a year old. Otherwise the form points to Form DS-82 or Form DS-11.

Will my documents be returned?

Documents you send that are not damaged, altered or forged are returned to you. Photocopies are not returned. Your new passport and your documents may arrive in separate mailings.

Will my new passport have the same number?

No. Each newly issued passport book or card has a different passport number from your previous one.
