Fill out Form G-1145 online

e-Notification of Application/Petition Acceptance

Form G-1145 asks U.S. Citizenship and Immigration Services (USCIS) to send you an email and/or text message when it accepts your application or petition. The service is only for certain forms filed at a USCIS Lockbox facility. The message gives your receipt number and tells you how to get case status updates.

The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

Who uses Form G-1145

  • Applicants filing an eligible form at a USCIS Lockbox, such as Form I-485, I-765 or N-400
  • Petitioners filing an eligible petition at a Lockbox, such as Form I-130 or I-751
  • People filing an eligible form who want an email or text with their receipt number in addition to the mailed receipt notice

Before you start

  • The immigration form you are filing, and a check that it is on the list of forms on the official G-1145 page
  • The full last, first and middle name of the applicant or petitioner
  • An email address you check regularly
  • A mobile phone number that can receive text messages, if you want a text (overseas customers receive email only)
  • A way to print the form, because it is clipped to your paper filing

How to fill out Form G-1145

  1. Check that your form qualifies

USCIS sends e-Notifications only for certain forms filed at its Lockbox facilities. The official G-1145 page lists the forms you can submit it with, including Forms I-90, I-130, I-131, I-485, I-765, I-751 and N-400.

  2. Enter the applicant or petitioner's name

Fill in the Applicant/Petitioner Full Last Name, Full First Name and Full Middle Name boxes.

  3. Enter your email address

Type the email address where you want the notice sent. Check it carefully: the form says undeliverable e-Notifications cannot be resent.

  4. Enter a mobile number for a text message

Fill in the Mobile Phone Number (Text Message) box if you also want a text. The form says domestic customers can get an email and/or text, while overseas customers receive only an email.

  5. Print and clip it to your filing

Print the form and clip it on top of the first page of the immigration form you are filing. The form says you will receive one email and/or text message for each form you are filing.

After you fill it out

Form G-1145 travels with your application or petition: clip it to the front of that form and send the whole package to the filing address listed for your main form. Check that address, and the fee, on the official USCIS pages. After USCIS accepts the package, it sends the e-Notification and also mails you a receipt notice (Form I-797C).

Signing: Form G-1145 has no signature line. The application or petition you clip it to has its own signature rules, and USCIS says a required signature must be handwritten, not stamped or typed.

Watch out for

  • The e-Notification does not grant any status or benefit. The form says to use the mailed receipt notice (Form I-797C) as proof of your pending application or petition.
  • Undeliverable e-Notifications cannot be resent, so check your email address and phone number before you print.
  • The service only works for certain forms filed at a USCIS Lockbox facility; check the list on the official G-1145 page.
  • If you print the form to mail it, USCIS says the edition date and page number at the bottom must be visible.

Questions about Form G-1145

What will the email or text message say?

It shows your receipt number and tells you how to get case status information. USCIS does not include personal information, including your name, because email and text messages are not secure.

Can I get a text message if I live outside the United States?

No. The form says overseas customers receive only an email, while domestic customers can receive an email and/or a text message.

Can I use an older edition of Form G-1145?

Yes. The official G-1145 page gives the edition date as 09/26/14 and says you can also use previous editions.

Is there a filing fee for Form G-1145?

The official G-1145 page sends you to the USCIS Fee Schedule page (Form G-1055) for the filing fee. Check it there before you file.

Does the e-Notification replace the receipt notice?

No. USCIS also mails a receipt notice (Form I-797C), and the form says to use that notice as proof of your pending application or petition.
