IVES Request for Transcript of Tax Return
Form 4506-C gives the IRS your consent to send your tax transcripts to a third party through the Income Verification Express Service (IVES). Banks, credit unions, mortgage lenders and other IVES participants use it to check income when you apply for a mortgage, loan or other service. It can be used by individuals, married couples who filed jointly, and businesses.
The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

How to fill out Form 4506-C
Lines 1a to 2c: Taxpayer names and numbers
On line 1a, enter your first name, middle initial and last name. For a business, enter the company name in the last name field. On line 1b, enter your SSN, ITIN or EIN with the dashes. On line 1c, enter the name on your last filed return if it was different. Fill in lines 2a to 2c only for a spouse on a joint return whose transcripts are also requested; that spouse must sign too.
Lines 3 and 4: Addresses
On line 3, enter your current street address, including any apartment or suite number, city, state and ZIP code. If you use a P.O. box, include it here. On line 4, enter the address on your last filed return if it is different. If the two addresses differ and you have not told the IRS about your move, the instructions say to file Form 8822 or Form 8822-B with Form 4506-C.
Lines 5a to 5d: Who receives the transcripts
Line 5a is the IVES participant: its name, 10-digit participant ID number, SOR mailbox ID and address. Line 5b (a customer file number of up to 10 digits) and line 5c (a unique identifier of up to 10 letters or numbers) are optional and must not contain an SSN, ITIN or EIN. Line 5d is the client company's name, phone number and address. It cannot be blank or marked NA; if the IVES participant is also the client, its details go on both 5a and 5d.
Line 6: Tax return transcripts
Enter one tax form number only, such as 1040, 1065 or 1120. Then check the transcript types wanted. 6a, Return Transcript, shows most line items of the return as filed. 6b, Account Transcript, shows payments, penalties and adjustments made after the return was filed. 6c, Record of Account, combines both. If you are not sure which type is needed, ask the party requesting your information.
Line 7: Wage and income transcripts
Check line 7 to request data from information returns such as Forms W-2, 1099, 1098 and 5498. On line 7a you can list up to three form numbers. To get all forms, leave it blank: writing NA or Not Applicable counts as an entry. On line 7b, check Line 1a, Line 2a or both to choose whose transcripts are sent. If neither box is checked, transcripts are sent for all listed taxpayers.
Line 8: Years or periods
Enter the end date of each tax year or period in mm dd yyyy format. It can be a calendar year, a fiscal year or a quarter, and there is room for four dates. For a calendar year 2018 Form 1040, the instructions give 12 31 2018 as the example. For quarterly returns, enter each quarter you want.
Sign Here
Check the attestation box above the signature line; the form is not processed without it. The taxpayer on line 1a signs and dates the form and fills in the phone number, the printed name and, if line 1a is a corporation, partnership, estate or trust, the title. A spouse listed on line 2a signs and dates the second signature area. Check the Authorized Representative box if a representative signs, and the electronic signature box only if the form is signed electronically.
After you fill it out
Return the signed form to the lender or IVES participant that asked for it. The participant faxes it to the IRS with an IVES cover sheet, or enters the request online so you can approve it in your IRS online account or business tax account. The IRS must receive the form within 120 days of the date you signed it, and the transcripts go to the IVES participant, not to you. Keep a copy of the request for your records.
Signing: Every taxpayer listed on lines 1a and 2a must sign and date the form after all applicable lines are filled in. Only IVES participants that are authorized for electronic signatures can accept one, so contact the participant first; otherwise print the form, sign it by hand and leave the electronic signature box unchecked.
Watch out for
- Do not sign until all applicable lines, including lines 5a through 8, are filled in. The IRS may reject an incomplete or illegible form.
- The IRS rejects Form 4506-C if it receives it more than 120 days after the signature date.
- Enter only one tax form number on line 6. A form that lists more than one tax form number is rejected.
- The IRS rejects a hand-signed form that has the "Signatory confirms document was electronically signed" box checked.
Questions about Form 4506-C
Will I get a copy of my transcripts from this form?
No. The IRS sends the transcripts to the IVES participant on line 5a, which may pass them to the client company on line 5d. To get your own transcripts, use your IRS online account, request them by mail through Get Transcript, or use Form 4506-T.
Can I refuse a request?
Yes. The IRS sends your tax records only if you approve the request. To reject it, select "Reject request" in your IRS online account or business tax account, or do not sign Form 4506-C.
Can my spouse and I use one form?
For a joint return, yes. List your spouse on line 2a only if the spouse's own transcripts are requested and the spouse will sign. People who file separately must each use a separate Form 4506-C.
Can someone else sign for me?
A representative can sign if you gave them this authority on line 5a of Form 2848 and Form 2848 is attached to the request. The representative must check the "Form 4506-C was signed by an Authorized Representative" box.
Does the IRS accept older versions of the form?
No. The IRS accepts only Forms 4506-C with a revision date of October 2022 or later.
