Request for Transcript of Tax Return

Form 4506-T asks the IRS to mail a tax transcript or a verification of nonfiling letter to the taxpayer's address of record. It covers return transcripts, account transcripts, record of account, verification of nonfiling, and transcripts of Form W-2 and Form 1099, 1098 and 5498 series data. Individuals and businesses use it, often when they need an older tax year or cannot get the transcript online.

The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

Who uses Form 4506-T

  • Individuals who need a transcript for a tax year not available online, or one their online account could not produce
  • People asked by a lender to verify past income
  • Businesses that need a business tax return or account transcript by mail
  • People who need a verification of nonfiling letter for an older year
  • Representatives the taxpayer has authorized on Form 2848, line 5

Before you start

  • Name and SSN, ITIN or EIN exactly as shown on the tax return (and your spouse's, for a joint return)
  • Your current address and the address on the last return you filed
  • The tax form number the transcript is for, such as 1040, 1065 or 1120
  • The end date of each tax year or period you need, in mm/dd/yyyy format
  • A customer file number, if a lender or other third party gave you one
  • For a business, estate or trust, or a representative: the authorization document, or Form 2848 showing the delegation

How to fill out Form 4506-T

  1. Lines 1a to 2b: Name and taxpayer number

On line 1a, enter the name shown on the return; for a joint return, use the name shown first. On line 1b, enter the EIN if the request is for a business return; otherwise enter the first SSN or ITIN on the return. For a joint return, enter your spouse's name on line 2a and the second SSN or ITIN on line 2b.

  2. Lines 3 and 4: Addresses

On line 3, enter your current name and address, including any apartment, suite or P.O. box. If you are incarcerated, include your inmate number. On line 4, enter the address on the last return you filed if it differs from line 3. If the two differ and you have not changed your address with the IRS, the instructions point you to Form 8822 (or Form 8822-B for a business).

  3. Line 5: Customer file number (optional)

You may enter up to 10 digits, such as a loan number a lender gave you. It will print on the transcript so the lender can match it to you. Do not use an SSN; if you enter an SSN or a name, the transcript shows 9999999999 instead.

  4. Line 6: Transcript of a return

Enter the tax form number, then check the box for the type you want: 6a for a return transcript (most line items as filed, without later changes), 6b for an account transcript (payments, penalties and later adjustments), or 6c for a record of account (both combined). Enter only one tax form number per request. The instructions say to choose the record of account if you are unsure which type you need.

  5. Lines 7 and 8: Nonfiling letter or information returns

Check line 7 for a verification of nonfiling, which is proof from the IRS that you did not file a return for that year; for the current year it is only available after June 15. Check line 8 for a transcript of Form W-2, 1099, 1098 or 5498 series data. State or local information is not included with the Form W-2 data.

  6. Line 9: Year or period

Enter the end date of each tax year or period you want, in mm/dd/yyyy format. For example, 12/31/2018 means the 2018 calendar year. For quarterly returns, enter each quarter separately.

  7. Signature area

Check the box confirming you have the authority to sign, then sign, date and add the phone number of the taxpayer on line 1a or 2a. Sign exactly as your name appeared on the original return, and also sign your current name if it changed. For a corporation, partnership, estate or trust, enter your title. For a joint return, only one spouse needs to sign.

After you fill it out

Mail or fax the signed form to the IRS. Page 2 of the form has two charts, one for individual transcripts and one for all other transcripts, that give the mailing address and fax number by the state you or your business were in when the return was filed; use the charts in the current version of the form linked from the official IRS page. The IRS mails the transcript only to the taxpayer's address of record, never to a third party, and it does not fax transcripts.

Signing: The taxpayer on line 1a or 2a, or a person with authority to act for them, must sign and date the form and check the attestation box, and the IRS must receive it within 120 days of that date. The instructions do not say whether an electronic signature is accepted, so follow the IRS instructions for this form.

Watch out for

  • Do not sign until every line that applies is complete. An incomplete or illegible form may be rejected.
  • The IRS must receive the form within 120 days of the date you sign it, or it will be rejected.
  • The form will not be processed if the box in the signature area is left unchecked.
  • The IRS does not mail transcripts to third parties such as lenders. They go only to the taxpayer's address of record.

Questions about Form 4506-T

Is a transcript the same as a copy of my return?

No. A transcript is not a photocopy of your return; it shows the tax information for the transcript type you request. To get a copy of the return itself, with its attachments, use Form 4506, Request for Copy of Tax Return.

Can I get a transcript without sending this form?

Often, yes. Individuals can view and download transcripts in their IRS Individual Online Account, or order a tax return or account transcript through Get transcript by mail or the IRS automated phone service. For a tax year that is not available online, the IRS says to submit Form 4506-T.

Why is some of my information hidden on the transcript?

To protect against identity theft, transcripts partially mask personal details such as names, addresses and taxpayer numbers. All money amounts stay fully visible.

What is the customer file number for?

It is an optional number of up to 10 digits, often a loan number from a lender. It prints on the transcript so the third party can match the transcript to you, since your full SSN is masked.

Can either spouse request a transcript of a joint return?

Yes. A transcript of a jointly filed return may be furnished to either spouse, and only one signature is required.
