Application for Replacement/Initial Nonimmigrant Arrival-Departure Document
Form I-102 asks USCIS for a new or replacement arrival-departure record: Form I-94, Form I-94W or Form I-95 (Crewman's Landing Permit). Nonimmigrants use it when their record was lost, stolen or mutilated, when they were not issued one at admission, or when USCIS issued one with wrong information.
The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

Who uses Form I-102
- Nonimmigrants whose Form I-94, I-94W or I-95 was lost, stolen or mutilated
- Nonimmigrants who were admitted at a port of entry but were not issued a Form I-94
- People whose Form I-94, I-94W or I-95 was issued by USCIS with incorrect information
- Nonimmigrant military members (U.S. Armed Forces, NATO, Partnership for Peace) who need an initial Form I-94
- People granted an extension of stay or change of status who need to replace a lost, stolen or mutilated Form I-94
Before you start
- If your record was lost, stolen or never issued: a copy of your passport's biographic page and of the page showing your admission, or other evidence of your admission
- A copy of a government-issued ID showing your legal name and date of birth
- Your Form I-94, I-94W or I-95 number, if you have it. For a mutilated record, the original form; for a USCIS error, the original form plus a signed, dated statement of what needs correcting and evidence of the correct information
- If your record or passport was stolen, a copy of the police report, or a written explanation of why there is none
- Evidence of any name change, if the name on your record differs from your current legal name
- A U.S. address: the instructions say you must have one to file this application
How to fill out Form I-102
Part 1, Items 1-11: Information About You
Enter your A-Number and USCIS Online Account Number if you have them (type "N/A" for an A-Number you do not have), your full legal name, and any other names you have used. Give your U.S. mailing address in Items 5.a-5.f, answer Item 6, and give your U.S. physical address in Items 7.a-7.f only if it is different. Finish with your date of birth, country of birth, country of citizenship and U.S. Social Security number, if any. If you have no citizenship, type "stateless" and explain in Part 7.
Part 1, Items 12-19: Entry Information
Give the date and place (city and state) of your last entry, your class of admission, and the type of port of entry (land border, airport or seaport). Enter your current nonimmigrant status and the date it expires, then your I-94, I-94W or I-95 number, passport number, travel document number, issuing country and expiration date. In Items 19.a-19.c, write your name exactly as it appears on the arrival-departure record.
Part 2: Reason for Application
Select only one box. Items 1.a-1.d are for a lost, stolen or mutilated Form I-94, I-94W or I-95. Item 1.e is for a record that was not issued when CBP admitted you at a port of entry. Item 1.f is for a record USCIS issued with an error; explain the error in the space under that box. Item 1.g is for a nonimmigrant military member who needs an initial Form I-94.
Part 3: Processing Information
Item 1.a asks whether you are filing this application with another petition or application; if yes, give that form's number and name in Item 1.b. Item 2.a asks whether you are now in removal proceedings; if yes, describe them in Item 2.b, using Part 7 if you need more room.
Part 4: Applicant's Contact Information, Certification, and Signature
Enter your daytime telephone number and, if you have them, a mobile number and email address in Items 1-3. Read the certification, then sign and date in Item 4. A parent or legal guardian may sign for an applicant under 14.
Parts 5 and 6: Interpreter and Preparer
Complete Part 5 only if someone interpreted the form for you, and Part 6 only if someone other than you prepared it. Each of them gives their name, organization and contact details and signs their own certification. If one person did both, that person completes both parts.
Part 7: Additional Information
Use this page for answers that did not fit. Put your name and A-Number (if any) at the top, and for each answer give the page, part and item number it belongs to. If you attach extra sheets, sign and date each one.
After you fill it out
Print the form, sign it in ink and mail it with your evidence and any required fee. Where you send it depends on your reason for filing and where you are located; if you file it together with another form, it goes to that other form's filing location. Check the current filing address on the USCIS Direct Filing Addresses for Form I-102 page and the fee on the USCIS Fee Schedule (Form G-1055) before you mail it. Military members of NATO or the Partnership for Peace under SOFA who are seeking an initial Form I-94 submit it through their foreign commander or that commander's designee.
Signing: USCIS will not accept a stamped or typewritten name in place of a signature, and it rejects an unsigned application. Print the form and sign it by hand in ink; a photocopy, fax or scan of the original handwritten signature is accepted.
Watch out for
- If CBP issued your Form I-94, I-94W or I-95 with incorrect information, do not file Form I-102. USCIS says to go to the nearest CBP port of entry or deferred inspection office to have it corrected.
- If you were admitted at an airport or seaport and received an electronic Form I-94, first try to get a paper copy from CBP's Form I-94 website.
- USCIS rejects the form if the family name, U.S. mailing address, date of birth or the Part 2 reason is missing, or if it is not signed.
- All pages must be from the same form edition, with the edition date and page numbers visible at the bottom of each page, or USCIS may reject the form.
Questions about Form I-102
Can I use Form I-102 to fix a mistake CBP made on my I-94?
No. Item 1.f of Part 2 covers records USCIS issued with an error. For an error CBP made, USCIS says to go to the nearest CBP port of entry or deferred inspection office.
Do I need Form I-102 if I just need a copy of my electronic I-94?
If you were admitted at an airport or seaport and received an electronic I-94, you can try to get a paper version from CBP's Form I-94 website. The instructions say Form I-102 can be used if you cannot get it there.
Can I still get a Form I-94W with this form?
Only in some cases. The instructions say a Form I-94W can be replaced or issued if you were admitted at a land border, or at an airport or seaport on or before June 28, 2010.
Who signs for a child?
If the applicant is under 14, a parent or legal guardian may sign the application. A legal guardian may also sign for a mentally incompetent person.
What evidence goes with the form?
Every application needs a copy of a government-issued ID showing your legal name and date of birth, plus the evidence for your reason in Part 2, such as passport pages, a police report or the damaged original form. Send copies, not originals, unless the instructions ask for the original. A document in a foreign language needs a full English translation with the translator's signed certification.
