Application to Replace Permanent Resident Card

Form I-90 is the USCIS application to replace a Permanent Resident Card (Green Card). Lawful permanent residents and permanent residents in commuter status use it to renew a card that has expired or will expire within six months, or to replace a card that was lost, damaged, never received, or shows wrong or outdated information. Conditional permanent residents can use it only to replace a card, and not when their card has expired or will expire within 90 days.

The form opens in our PDF editor with its own fields ready to type into. You can fill it in without an account; downloading the filled PDF needs a free account. A free account gets 15 credits a day, enough for one form a day.

How to fill out Form I-90

  1. Part 1. Information About You

Enter your A-Number (Item 1), USCIS Online Account Number if any (Item 2), and full legal name (Items 3.a.-3.c.); the new card is issued in this name. If your name legally changed since the card was issued, answer Yes in Item 4 and give the name exactly as printed on your current card in Items 5.a.-5.c. Give your mailing address in Items 6.a.-6.i., your physical address in Items 7.a.-7.h. only if it is different, and your personal details in Items 8-16: sex, date and place of birth, parents' given names, class of admission, date of admission, and Social Security number if any.

  2. Part 2. Application Type

Select one box in Items 1.a.-1.c. for your status: lawful permanent resident, permanent resident in commuter status, or conditional permanent resident. Lawful and commuter residents then select one reason in Section A (Items 2.a.-2.j.); conditional residents select one reason in Section B (Items 3.a.-3.e.). If you select 2.h.1. (taking up commuter status), give the U.S. Port-of-Entry you will use in Item 2.h.1.a. Each reason in the instructions lists the evidence to send with it.

  3. Part 3. Processing Information

Items 1 and 2 ask where you applied for your immigrant visa or adjustment of status, and where the visa was issued or the adjustment granted. Complete Items 3.a. and 3.a.1. only if you entered the United States with an immigrant visa. Item 4 asks about exclusion, deportation, or removal proceedings or a removal order, and Item 5 about Form I-407 or abandonment of status; explain any Yes answer in Part 8. Items 6-11 ask for ethnicity, race, height in feet and inches, weight in pounds, and eye and hair color.

  4. Part 4. Accommodations

Answer Item 1 to say whether you are asking for an accommodation because of a disability or impairment. If Yes, select the boxes in Items 1.a.-1.c. that apply and describe what you need.

  5. Part 5. Applicant's Statement, Contact Information, Certification, and Signature

Select Item 1.a. if you read and understood the form in English, or Item 1.b. if the interpreter in Part 6 read it to you, and Item 2 if a preparer helped you. Give your daytime phone number in Item 3, and your mobile number and email in Items 4 and 5 if you have them. Sign Item 6.a. in ink and write the date in Item 6.b.

  6. Parts 6 and 7. Interpreter and Preparer

Fill these in only if someone interpreted the form for you (Part 6) or prepared it for you (Part 7); one person who did both completes both parts. Each gives their name, organization if any, address, and contact details. The interpreter signs and dates Items 7.a.-7.b. of Part 6, and the preparer selects Item 7.a. or 7.b. and signs and dates Items 8.a.-8.b. of Part 7. An attorney or accredited representative whose role goes beyond preparing the form may also need to file Form G-28.

  7. Part 8. Additional Information

Use this page for any answer that does not fit, including explanations of Yes answers in Part 3. Give your name and A-Number at the top, list the page, part, and item number for each answer, and sign and date each extra sheet.

After you fill it out

The completed form goes to USCIS with the evidence for your reason and the correct fee, if one applies. You can mail the paper form to the USCIS address shown on the official Form I-90 page; USCIS also accepts Form I-90 filed online through a USCIS online account, which is a separate process on the USCIS website. Check the current filing address, fee, and accepted payment methods on the official Form I-90 and Fee Schedule pages before you send it. USCIS may then mail you a biometrics appointment notice; if you move while the case is pending, update your address with USCIS.

Signing: Item 6.a. says to sign in ink, and USCIS does not accept a typed or stamped name in place of a signature, so print the form and sign it by hand. A parent or legal guardian may sign for an applicant under 14, and USCIS accepts a photocopy, fax, or scan of the original hand-signed form.

Watch out for

  • If you are a conditional resident and your status expires within the next 90 days, do not file Form I-90. The instructions say to file Form I-751 or Form I-829 to remove conditions instead.
  • USCIS rejects Form I-90 if the family name, mailing address, or date of birth in Part 1 is missing, and rejects any unsigned form.
  • All pages must be from the same edition (01/20/25), with the edition date and page numbers visible at the bottom. USCIS may reject the form if a page is missing or from a different edition.
  • If you file because of a DHS error on your card (reason 2.d. or 3.d.), you must send the original card. A copy is not accepted for that reason.

Questions about Form I-90

Can I use Form I-90 to renew a Green Card that is about to expire?

Yes, if you are a lawful permanent resident or a permanent resident in commuter status whose card has expired or will expire within six months; you select reason 2.f. in Part 2. If you choose that reason and your card will not expire within six months, USCIS may deny the application.

Can a conditional resident with a two-year card use this form?

Only to replace a card for a reason in Part 2, Section B: lost, stolen, destroyed, mutilated, never received, a DHS error, or a legal change of name or other details. A conditional resident whose status expires within 90 days files Form I-751 or Form I-829 instead.

My card was mailed but never arrived. Which reason do I select?

The never-received reason (2.b. or 3.b.) applies only if the card was returned to USCIS as undeliverable, which you can check through your case status. If it was not returned, the instructions say to use the lost, stolen, or destroyed reason (2.a. or 3.a.).

What proves my status while the application is pending?

If you are renewing, the Form I-90 receipt notice together with your expired card serves as evidence of your status for the period stated on the notice. If you no longer have a card, USCIS may issue an ADIT stamp after you file.

Will I need to attend an appointment?

USCIS may require a biometrics appointment to take your fingerprints, photograph, and/or signature, and will mail you a notice with the date, time, and place. At the appointment you sign an oath confirming that the information in your application is complete, true, and correct.
